6. Suchtaspekte bei Adipositas

Zusammenfassung

Sowohl auf phänomenologischer als auch auf neurobiologischer Ebene bestehen Gemeinsamkeiten zwischen bestimmten Aspekten der Adipositas und klassischen Suchterkrankungen. Adipöse wie Suchtkranke führen süchtiges Verhalten oft trotz Wissen um die negativen Konsequenzen fort und Verhaltensänderungen führen meist zu Rückfällen in alte Verhaltensmuster. Zu berücksichtigen ist allerdings, dass die Belohnungsfunktion von Nahrung auch von homöostatischen Mechanismen beeinflusst wird, insbesondere dem Sättigungsgrad. Im vorliegenden Artikel werden die neurobiologischen Gemeinsamkeiten im Zusammenhang mit dem mesolimbischen, dopaminergen Belohnungssystem diskutiert; bildgebende, neuroendokrine und genetische Untersuchungsansätze dargestellt. Die zunehmende Erhöhung der Inzidenz der Adipositas macht es notwendig, innovative Therapieansätze zu definieren. Eine simplifizierende Differenzierung „Sucht: Ja/Nein“ ist bei einem komplexen Krankheitsbild wie der Adipositas nicht zielführend.
