We thank Liebert et al. for their interest in our study on radiological apical tumor involvement (radATI) and its impact on clinical outcomes such as apical positive surgical margins (APSM) and biochemical recurrence (BCR) [

2]. Liebert et al. argue that the literature suggests that a single APSM may be clinically insignificant for long-term outcomes. Therefore, a more radical apical dissection when radATI is present may not affect outcome and lead to overtreatment with potential impairment of functional outcome. …
