12. Lymphadenektomie bei Magenkarzinom und AEG

Zusammenfassung

Das Ausmaß der Lymphadenektomie beim Magenkarzinom gehört zu den in den beiden vergangenen Dekaden am häufigsten diskutierten Problemen in der Magenchirurgie. Im Fokus steht dabei die Frage, ob die in Japan als Standardprozedur durchgeführte D2-Lymphadenektomie in westlichen Patientenkollektiven gegenüber der D1-Lymphadenektomie einen vergleichbaren Überlebensvorteil bei niedriger Morbidität mit sich bringt. Die Anwendung endoskopischer Resektionsverfahren für Frühkarzinome erfordert es, in Abhängigkeit von der Invasionstiefe des Tumors Patientengruppen zu identifizieren, bei denen das Risiko einer Lymphknotenmetastasierung gering ist. Der zunehmende Einsatz laparoskopischer und auch roboterassistierter Verfahren insbesondere bei distalen Tumoren und Frühkarzinomen, aber auch bei lokal fortgeschrittenen Karzinomen wirft die Frage auf, ob die minimalinvasiv durchgeführte Lymphadenektomie hinsichtlich Sicherheit und onkologischem Ergebnis dem offenen Verfahren gleichwertig ist.
