4. Grundlagen der Transplantation

Zusammenfassung

Die Fettgewebstransplantation dient kosmetischen wie auch rekonstruktiven Zwecken. Die Methoden der Entnahme, Aufbereitung und Transplantation variieren in der Praxis. Transplantate werden beinahe ausschließlich mittels spezieller Entnahmekanülen entnommen. Kleinere Fettpartikel werden durch feinere Kanülen mit kleineren Durchmessern der Seitöffnungen gewonnen und genauso wieder mit dünneren Injektionskanülen transplantiert. Es besteht ein erwiesener Zusammenhang zwischen dem Durchmesser des Fettpartikels und dem der Löcher in der Entnahmekanüle. Das Fettgewebe kann an verschiedenen Körperregionen entnommen werden, meist in der Gesäß- oder Trochanterregion, aber auch die Innenseiten der Oberschenkel oder die Knieregion sind möglich. Bereits wenige Stunden postoperativ ist die beginnennde Neovaskularisation nachweisbar. Ein gewisser Volumenverlust ist die Regel. Zuletzt werden Risiken und Komplikationen der autologen Fettgewebstransplantation dargestellt.

Bei schwerer Aortenstenose und obstruktiver KHK empfehlen die Leitlinien derzeit eine chirurgische Kombi-Behandlung aus Klappenersatz plus Bypass-OP. Diese Empfehlung wird allerdings jetzt durch eine aktuelle Studie infrage gestellt – mit überraschender Deutlichkeit.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
