Arthrogene Muskelinhibition (AMI)

Stellenwert der Prävention und Therapie nach Kniegelenkseingriffen

Zusammenfassung

Charakteristika der arthrogenen Muskelinhibition (AMI) sind ausgeprägte Aktivierungsdefizite und Kraftverlust der Muskulatur, v. a. des M. quadriceps. Sie manifestiert sich meist in der Akutphase nach Trauma oder Operation, mit einer postoperativen Verringerung der Muskelaktivität über mehrere Tage und Wochen. Ihre Genese ist multifaktoriell. Eine bilaterale AMI ist auch bei unilateralem Trauma möglich, was eine supraspinale Komponente derselben nahelegt. Die frühzeitige Überprüfung der Ansteuerbarkeit des M. quadriceps spielt für die Prognose der AMI eine entscheidende Rolle. Im postoperativen Verlauf sollte sie ebenfalls regelhaft kontrolliert werden, um ggf. weitergehende Diagnostik und gezielte Interventionen einzuleiten. Therapeutische Optionen reichen von Kryotherapie, Blutflussrestriktionstraining, neuromuskulärer Elektrostimulation und Ultraschall in der Akutphase bis zu Bewegungsvorstellungs- und exzentrischem Training oder dem Einsatz virtueller Realität.
